Privacy of information relating to the account holder, additional cardholder and supplementary cardholder(s) (“activa customers”) and the activa Plan policy holder and its dependants  (“Southern Cross Members”) is governed by the Privacy Act 1993 and, in relation to health information, by the Health Information Privacy Code 1994.

  1. Activa Health Limited (“Activa”), ASB Bank Limited (“ASB”) and Southern Cross Medical Care Society (“Southern Cross”) will collect personal information about activa customers and Southern Cross Members.
  2. Activa and Southern Cross will also collect health information about the Southern Cross Members.
  3. The information Activa, ASB and Southern Cross collect will be used for the following purposes (the “Purposes”):
    • to consider the account holder’s eligibility to open and operate an activa Account with ASB; 
    • to verify any activa customer’s identity;
    • to monitor the account holder’s ongoing creditworthiness;
    • for administrative purposes, such as billing and claims management;
    • by a credit reference agency or debt collection agency in the event of breach of the terms and conditions by any activa customer;
    • to contact any activa customer and/or Southern Cross Members from time to time with information related to Activa;
    • for market research and targeted promotions by Activa;
    • for statistical purposes (although for this purpose the relevant person(s) will not be identified).
  4. The intended recipients of the information are:
    • Activa;
    • ASB (other than health information);
    • Southern Cross;
    • Activa’s agents, contractors, suppliers and other business partners, but only to the extent necessary for the Purposes.
  5. The information will be collected and held by:
    • Activa, Level 1, Quay Tower, 29 Custom St West, Auckland.
    • ASB, ASB Bank Centre, 135 Albert Street, Auckland.
    • Southern Cross, 181 Grafton Road, Newmarket.
  6. The information may be collected from activa customers and Southern Cross Members directly (for example, through the application process), and may also be collected from activa customers by how the activa Account and the related activa card(s) are used.
  7. Activa and ASB may also collect and disclose information about the account holder from and to credit reference agencies and credit providers for any of the Purposes (and the account holder authorises this collection by Activa and ASB and disclosure by and to credit reference agencies and credit providers).
  8. Activa may also collect and disclose information about the activa customers from and to activa Partners for market research and targeted promotions (and the activa customers authorise this collection by Activa and disclosure by and to the activa Partner).
  9. Activa and Southern Cross may also collect and disclose information about the Southern Cross Members from and to healthcare services providers and medical authorities for any of the Purposes (and the Southern Cross Members authorise this collection by Activa and Southern Cross and disclosure by and to the health services providers and medical authorities).
  10. All communications from Activa, Southern Cross or ASB relating to any activa customer or Southern Cross Member will be sent to the account holder.  This means that information about each activa customer and Southern Cross Member will be disclosed to the account holder (and each of the activa customers and Southern Cross Members authorises this disclosure).
  11. Failure to provide any requested information or provision of incorrect information may result in any application for credit being declined, or an activa Account not being opened.
  12. Under the Privacy Act and the Health Information Privacy Code, the activa customers and the Southern Cross Members are entitled to have access to, and request correction of, any personal information (including any health information) held by Activa, ASB and Southern Cross.  If any activa customer or Southern Cross Member has any queries about privacy of information or wish to access any personal information held by Activa, ASB or Southern Cross, please contact activa Customer Care on 0800 228 482.
  13. Activa, ASB and Southern Cross will endeavour to ensure that the information that they hold about any activa customer and any Southern Cross Member is accurate. Prompt notification of any changes in personal contact details such as residential or email address, or telephone or facsimile numbers will help them to do this.

Tips for Maximising Your Security

 

Password security

 

At activa we take practical steps to ensure that our website is secure, but there are risks involved in using activa Online services and we recommend all customers should also take their own steps to increase their level of protection.

Below we've suggested a few ways you can protect yourself.

  • Choose a password you will remember, but that is not easy to guess. It must include some numbers as well as letters, and possibly other special characters.
  • When entering your password please take care so that others cannot see what you are entering.
  • We recommend that you change your password regularly. You can do this within the activa Online area once you have logged in.
  • Do not write your password down or leave it in a place where others may see it - you should commit it to memory only. If you do forget it, simply use the ‘Reset My Password’ link or call the activa customer care team, who can create a new temporary password for you. If you believe someone may know your password you should change it immediately.
  • You should never tell anyone your password, even if requested. Our staff will never ask you for your password under any circumstances.
  • Do not leave your computer unattended when logged in to activa Online.
  • Always use the Logout button when you have finished with activa Online. As a security measure, if there is no activity in your activa Online session for 30 minutes, you will be automatically logged out.
  • If your browser prompts you, do not save your login details.

 

Internet and PC security

  • We suggest you avoid using computers that you think might pose a security risk to access your activa Online, such as Internet Cafes. These computers could be running programs that record the keys you type and potentially put your password and other personal information at risk
  • Install anti-virus/anti-malware software on your computer and keep it up-to-date.
  • Install a personal firewall on your computer.
  • Keep your browser up-to-date with security patches.
  • To ensure you are on the official activa website, never click on an emailed link. Always login to activa Online by typing www.activa.co.nz into your browser’s address bar or by using your favourites/bookmarks.
  • Make sure the padlock symbol, indicating a secure environment, appears at the bottom of your browser, or look for the secure ‘https://’ prefix instead of the insecure ‘http://’ at the start of the web address.
